This is a speed entry screen to allow you to update information about patients that have symptoms of COVID-19. Once a patient is reported to have symptoms, update the patient as the symptoms progress or the patients recover.

You do not have to enter information into all fields for a patient at one time. Only enter information in the fields that apply at the time you are updating the patient’s record. The patient’s case is considered “Resolved” when the patient has recovered or passed away. If a patient is discharged prior to you knowing the outcome you must include the last known information, even for discharged patients.
