PDMP Consent / Authorization Status Report

This report will only include active patients and will display the consent status of patients to release information to the State’s Prescription Drug Monitoring Program (PDMP).

Instructions

This report will show the PDMP consent status for all active patients at one or more sites. Consent is automatically revoked upon discharge from treatment. Patients can also revoke consent at any time, and depending on the date the report is generated, data may change.
